It is our policy at Provident Orthopedic and Sports Medicine to verify your current or updated insurance benefits before each of your visits.

For many people this means that your co-pay, co-insurance, deductible, and out of pocket costs will start over. Your portion of the co-pay, co-insurance, deductible and maximum out of pocket fees will be collected at the time of your appointment. If you have questions, please let us know.
