In order to better serve you, please do not print out and fill out this webpage paperwork unless you are directed to do so by the front desk. We provide this as a reference for you as to the most pertinent information that each patient must have gathered and input in our Ipads before being seen. We exclusively utilize our portal. When you provide a working email we will send a link to that email and you can begin the registration process using a link. The link expires after more than 48 hours so you will need to call the office to activate another link if your time exceeds that.

The portal link to your email is provided in order to save you and your family time in the office before being seen by the physician. Please bring any additional info that will be helpful to render the best care including any hospital admission info and imaging reports and CDs with images. Please notify the front desk and the medical assistant as to where the images were obtained and when. Having these resources in advance will allow Dr. Shuler to provide the very best care for you and your loved one. It often takes at least 2 or 3 business days for an imaging report and images such as Xrays to be made available in a portal and longer for MRI imaging.
